You Must:

  • Have a recent Aramark Healthcare receipt from a participating store.

Instructions:

  2. If you prefer to take the survey in Spanish, click the green button to the left of the page. To take the survey in French, click the turquoise button, which is also to the left. If you prefer to take the survey in English, proceed to Step 3.
  3. If you have a receipt, click the receipt button; otherwise, click the invitation button.
  4. Enter the survey code, amount and date of visit, then click enter.
  5. Answer questions honestly and thoroughly. These questions will rate your experience and can use text boxes and multiple choice questions. It is important to follow all directions, as each section may be slightly different. You can close your browser when finished.
