  2. For English, go to step 3. For Spanish, click the hyperlink in the left bottom corner.
  3. Enter the survey code of 14 digits from your receipt into the text box provided. This can be found in the middle of your receipt. If you need to see an example, click the hyperlink under the start button. Click start when ready.
  4. Answer the questions that will span over the next few pages. Answer them honestly and take as much time as you need.
  5. After the questionnaire, fill out the form to enter the sweepstakes. If you don’t want to enter the drawing, close your browser window. You are not required to enter the sweepstakes to leave feedback.
