  3. You will be asked a series of questions based on your most recent visit to Dollar Tree. Answer these as honestly and thoroughly as possible.
  4. At the end of the survey, you have an opportunity to fill out a form for a chance to instantly win $1500 weekly and ten chances to win $1,000. Fill out this form accordingly.
  5. Once the form has been submitted, you will be notified if you have won the iPod. Follow instructions if you have won.
