Steps To Do Survey:

- Enter the 18-digit code found at the bottom of your receipt. Click start to begin.
- You will be asked questions about your most recent visit. Please answer these as honestly and as thoroughly as possible. Answer all questions on one page before moving to the next page. Some questions will offer a text box to elaborate. Please use this whenever possible to give a better response.
- At the end of the survey, you will be asked to enter the sweepstakes. If you do not wish to enter, please close your browser window. If you want to enter, please type in your contact information as requested. Read the directions.
