How to Participate in the Sephora USA Survey

You Will Need:

  • Have your recent Sephora USA receipt that contains an invitation for the survey.
  • Be 18 years of age or older to participate.

  6. Please answer the questions as they are shown. Answer all questions on the page and click next when ready.
  7. When you have finished the survey, you will be asked if you want to enter the sweepstakes. Select yes or no and click finish. If you select yes, you will be asked for contact information. If you select no, you will be asked to close the browser window.
