Steps To Do Survey:

- Before taking this survey, make sure you have your eyewear already, as some questions will pertain to feel and usability of the purchase. Click start when ready.
- Enter the ID number from your receipt. Click the hyperlink for an example. Before clicking continue, you must read the contest rules and check the box that says you have read them. Then you can click Continue.
- Answer the questions. You must answer all questions before you can move on. Some questions will be multiple choice and others will provide text boxes. Please follow directions.
