You Will Need:

- Have a recent Chevys receipt from a participating store.
Steps To Do Survey:
- Go to www.Chevysfeedback.com.
- Select your preferred language by clicking on it.
- Enter the code from the middle of your receipt. This should be 16 digits and there is a diagram to the left of the page that can help you more. Click Take Survey when ready.
- Answer questions rating your experience. Read all of the directions before beginning each page or section. Be honest and thorough.
